GENERAL DESCRIPTION SUMMARY
The Battle Buddy Community Health Worker is the foundational, “boots-on-the-ground”, field position for Hero’s Bridge—from initial assessment at first contact, through determination and facilitation of other programs and community assistance, to the final data analysis and review of veterans’ outcomes.
Essential Duties & Responsibilities
1. Assists leadership to build, implement, support and grow Battle Buddy services within assigned area.
2. Learn and embrace the Community Health Worker model as the basis of the Battle Buddy relationship.
3. Learn and embrace the four Core Values of Hero’s Bridge and reflect regularly on your interactions for adherence
4. Implement day-to-day operations of Battle Buddy goals, objectives and visit plans.
5. Collaborate and assist in engaging community partners, healthcare entities, Veteran Affairs personnel and other Veteran Service Organizations.
6. Complete an initial assessment on each referred veteran and plan visits and interventions to incrementally improve their quality of life.
7. Adhere to program objectives by recording information, data collection and analysis.
8. Monitor program effectiveness to the program goals and improvement of veteran QOL
9. Oversees and ensures documentation of all assessments, visits and interventions performed with veterans into appropriate software application.
10. Represents Hero’s Bridge providing responsive, courteous and respectful communication, exhibiting excellent service for all Hero’s Bridge relationships.
11. Coordinate with other Hero’s Bridge team members and programs to achieve positive outcomes.
12. Perform joint visits with HB volunteers to provide orientation as well as facilitate a smooth transition of services from paid staff to volunteer visits.
13. Provide meticulous follow up to veterans, families, referral sources and community partners to avoid perception of service delays, wait lists and other difficulties rampant at other VSO’s.

15. Assist in staffing of the HB office during business hours
Requirements:
At least 2 years of experience working with veterans, the elderly or health care partners
Demonstrated proficiency in MS Word and Excel as well as basic internet and social media use
Must be able to pass a background check
Valid Virginia driver’s license and reliable transportation
Effective written and oral communications skills

Travel/Office/Remote
This is a community-based position that requires significant daily local travel within the Piedmont region of Virginia. This position is estimated to be making in person visits and driving at least 60% of the week, in the office 20% and remote 20%. This blend is subject to change based on the needs of our veterans and the organization.
Physical Requirements
This position requires full range of body motion. While performing the duties of this job, the contractor is occasionally required to walk and stand for long periods of time; talk or hear, both in person and by telephone; use hands repetitively to handle, feel or operate standard office equipment; reach with hands and arms; and lift to 50 pounds. There will at times be activities in homes of veterans in less than ideal living conditions.
